STATE HEALTH
CRITERIA
For hospitals working to provide services for Sehat Sahulat Program of the contract, the following criteria would be applicable:
1.) Hospitals must have 10 beds and must have a valid provisional license issued by Punjab Health Care Commission.
2.) Hospitals must have 24/7 emergency services
3.) All general hospitals must have at least 4 departments including Emergency, General Medicine, General Surgery and Gynecology Services. However, exception can be given to single specialty hospitals including but not limited to cardiology, dialysis, ophthalmology, cancer, thalassemia and orthopedics subject to any amendment by the Client.
4.) Cardiac setups must have 24/7 emergency, CCU and interventional and cardiac surgery services.
5.) Hospitals are assessed on the basis of Standard Empanelment Criteria
6.) On the basis of scores secured during assessment, hospitals shall be graded on the formula given hereunder. The grade of the hospital in a specific district / rural tehsil shall determine whether the hospital is eligible for empanelment and subsequently for the contractual treatment rate between SLIC and the hospital

7.) In case any service provider fails in the assessment, the revised assessment would be done after 6-12 months subject to the need for new empanelment in specific area.
1.) The district wise demand of hospital will be estimated by State Life to assess supply and demand of service provider in the specific region.
2.) A portal is established where interested service providers can sign up and apply for empanelment through self-assessment.
3.) Hospitals obtaining the minimum required assessment score (based on self-assessment) would be considered for physical assessment based on active demand of hospitals in the respective region / district.
1.) A knockout criterion would be used to shortlist the interested hospitals which is as follows:
Hospital must be provisionally licensed with PHC
Hospital must have 10 beds (3 to 5 feet apart with proper ventilation)
Hospital must have minimum number of required departments (where required)
Hospital must have availability of consultant of required departments that must work at least 3 days (3 Hours) OPD at respective Hospitals.
Hospital (Multi specialty/General) must have 24/7 emergency services as per Clauses 2 & 4 mentioned under Criteria section and adequate number of Mos/PGs trainees (at least 1) including one Female MO in each shift.
Hospital providing gynae services must have availability of Separate Labor Room, In-house or On call Paedatrician and CTG machine.
Hospitals providing Ophthalmology must have separate eye operation theatre.
Hospitals providing Orthopedic services must have separate orthopedic operation theatre.
Hospitals performing cardiac surgery must have separate operation theatre. CCU will be mandatory for such hospitals.
Hospital must have availability of Pharmacy Services with qualified pharmacist Services (Owned or contracted out)
Hospital must have availability of nursing staff with at least 1 qualified nurse
Hospital must have availability of Labs and Diagnostic Services (Owned or contracted out)
Hospital must have basic infrastructure facilities including
X-Ray (Owned or contracted out)
USG
Availability of in-house/on call Sonologist
Availability of in-house/on call technicians
Availability of OT, X-Ray, LAB, ECG (Owned or contracted out)
2.) This knockout criterion (Clause 7) would be used to shortlist the interested hospitals for further detailed assessment by the organization.
